Complete Your 2019 Annual Enrollment Discontinuation of Healthcare and Related Benefits Form

In an effort to make sure all those who DO NOT participate in National Fuel's Insurance Benefits get their "one-time taxable payment" at the end of 2019, please make sure your signed and completed "2019 Annual Enrollment Discontinuation of Healthcare and related benefits" form (indicating you are opting out of receiving insurance benefits through National Fuel) is submitted and received at Human resources no later than November 14th, 2018. You should have received the form in the mail at your home address along with the rest of your benefits package paperwork.
 
Also, IT IS IMPORTANT that you make a copy of your completed form should any discrepancies or confusion arise. This way you will have a copy as proof of your completed document should the need present itself to show it to Human Resources if they should misplace or not receive it for any reason.
